Please complete this form if your child attends The Basilica School of Saint Mary or another Catholic School such as The Heights, Oakcrest School, St. Anselm Abbey, Angelus.
Homeschooling Families
For families who are homeschooling, with permission from the Director of Religious Education, a Diocesan-approved curriculum must be used for sacramental preparation of children who are preparing for First Reconciliation and First Holy Communion.
This registration is generally not for students enrolled in our children’s faith formation/CCD program in year 2 as we usually collect this information and fees when you enroll in the Sunday program for year 2.
