Marriage Preparation Questionnaire

Engaged couples must complete this form before they are able to schedule an initial marriage preparation meeting with a priest of St. Mary’s. Please fill out this form honestly and completely.

  • Please enter either the bride's, the groom's, or either set of parents' parish ID number. If you don't remember your parish ID number or are not registered parishioners, please contact the parish office at 703-836-4100 or admin@stmaryoldtown.org.

  • Please indicate the name, city, and state of the church.
  • Please note that his availability is not guaranteed and you may need to work with another priest of St. Mary's.
  • Please note that if your wedding is to take place at a different church (not at the Basilica), our priests do not travel for weddings. You must arrange for a more local priest to celebrate your nuptial Mass.
  • Please note that any priest outside of the Diocese of Arlington will need to request that a letter of suitability be sent to our parish office at 313 Duke Street prior to the wedding.
  • Please refer to the Basilica's wedding guidelines for more information on the different types of weddings.
  • Please note that the date(s) you indicate on this form may or may not be available depending on the priests' and the church's availability. Couples are responsible for coordinating scheduling with the priest preparing them for marriage and the parish office. Please note that although weddings are permitted during Lent, it is a penitential season. There will be no flowers, and the statues and crucifix will be covered in a deep purple cloth. Accommodations will not be made to alter this penitential state.
