Funeral Liturgy Checklist

If you have scheduled a funeral at the Basilica of Saint Mary, please use this form to indicate your selections for the funeral liturgy. This information will be passed along to the priest celebrating the funeral and to the Director of Music who will assemble the funeral programs. Please refer to the Basilica’s funeral planning guidelines for the full text of all the readings options listed in the drop-down menus below. While this form is necessary to construct the programs, families who have scheduled funerals with a visiting priest should be aware that we can only forward this form submission onto the celebrant of the Mass if his name and email are provided at the end of the form.

Readings
Please select the Scripture passage below and indicate the name of the person who will read it. Persons participating as readers must be practicing Catholics.

Prayer of the Faithful
The Prayer of the Faithful is prepared by the priest and read by a reader selected by the family. The reader must be a practicing Catholic. Please indicate the name of the reader here.

Gift Bearers
The gift bearers will bring the gifts of bread and wine from the back of the church to the priest during the Offertory prayers which are after the Prayer of the Faithful. Persons participating in this role do not need to be practicing Catholics.

Pall Bearers
The pall bearers will assist with bringing the casket into the church and with carrying the casket out of the church. Persons participating in this role do not need to be practicing Catholics. If there will be an urn burial, you may leave this section blank.

Visiting Priests
If you are working with a visiting priest as the celebrant of the funeral Mass, please provide his name and email in the spaces below. You must provide these details in order for the parish office to forward your submission of this form onto the celebrant of the Mass. You are otherwise responsible for communicating your preferences directly to the priest. If you are working with a priest of the Basilica of Saint Mary, you may leave this section blank.
